To provide the best opportunity to fund your loan request, please submit all required documents electronically in a legible and concise format, including documents you believe CNB Bank may already have.  This is an effort to expedite the process as we are dependent upon one system within the Small Business Administration, throughout the country, to submit all PPP requests.  You are encouraged to consult your tax preparer / CPA to help assemble the information. Your tax preparer / CPA may summarize the information and provide CNB Bank, however the supporting documentation below is required:
  1. Paycheck Protection Program Application SBA Form 2483 and Supplemental Sheet (Attached)
  2. Articles of Organization / Incorporation
  3. SS4 (IRS form showing the entity’s EIN number)
  4. Copy of current photo ID of all owners owning 20% or more of the company
  5. 2019 IRS Quarterly Payroll Tax Reports (940, 941 or 944)
  6. 2019 Quarterly State Unemployment Reports (with Quarterly Wage report)
  7. Documentation showing total of all Health Insurance Premiums paid by company in 2019.  Include all employees and the company owners.
  8. Documentation showing all Retirement Plan funding paid by company in 2019.  Include all employees and the company owners. Do NOT include the funding of employee deferrals from paychecks.
  9. 2019 IRS Form 1040, Schedule C – For applicants with self-employment income, irrespective of the 2019 filing status (see PPP IFR 4/14/20 Overview 1.b.iv., page 6).
